DIRECTIONS FOR CREATING A CHILD’S EDUCATIONAL SUPPORT FILE
ITEMS YOU WILL NEED:
· 1 - Expandable file pocket folder, letter size, at least 1 ½” expandable
· 5 - letter size divider sheets (card stock strength) (with tabs to label at top)
STEP 1:
· Obtain 5 pages of printed material from “It Takes A Village” Parent Support Group.
· In the following order, selecting divider with tab all the way to the left, glue the printed page to the front of a divider page.

#4 begins IEP DOCUMENTS
#5 begins THE TRANSITION PROCESS
STEP 2:
· At the top of each divider page, *if you have a notched label area, you can prepare an Avery label or simply write the following information:
#1 GENERAL INFORMATION
#2 HOME AND SCHOOL COMMUNICATIONS
#3 ASSESSMENTS & EVALUATIONS
#4 INDIVIDUALIZED EDUCATION PROGRAM
#5 TRANSITON
*NOTE – if your divider pages do not have a notched label area, you can affix label tabs to the top and insert the information behind the clear plastic label area.
STEP 3:
Place your child’s information behind each divider page with most current information forward. (working backwards in date order)
SUGGESTION: You may want a second expandable folder for this, and is very helpful for the child who struggles with organization and turning assignments in on time.
Keep your child’s graded class work through the entire 9 weeks, stapling each subject’s assignments weekly. That way, if the teacher did not have a grade in her book, you can refer to your folder. As your child gets older, you may advise your child to keep their assignments for the entire semester so the child can prepare for final exams.
